Medical Case Management
If you're an Apicha patient experiencing unexpected challenges like a change in income, health insurance issues, or a sudden medical diagnosis, our Medical Case Managers (MCMs) are here to assist you.
Our MCMs specialize in coordinating services and connecting you to external programs that offer vital support. Whether it's enrolling in SNAP benefits, finding new insurance, or accessing housing resources, our dedicated team is ready to guide you through these processes.

Case Management Programs
Our case managers are here to help you apply for public benefits and entitlements. We offer a welcoming and secure environment for individuals in various situations, including:
- those who are living with HIV/AIDS
- individuals seeking PrEP
- people who identify as Transgender or Gender Nonbinary.
When you join our case management program, you'll be assigned a dedicated case manager and, if needed, a bilingual assistant. Together, they will assess your needs and assist you in accessing services such as medical treatment, mental health support, health insurance, public financial assistance, housing solutions, and various other supportive services. Your case manager will maintain ongoing communication with you to continually assess and address your social services needs.
Our services are confidential and provided at no cost, regardless of your gender, sexual orientation, immigration status, nationality, religion, or creed.
